VHS is looking for a qualified Nurse Practitioner - Family Medicine.

• Description : Candidate CVs need to have month & date on their work experience w / all gaps in employment accounted for.Chart closure within 72 hours Schedule = 18 PPD, minimum is 18 PPDMonday-Friday 8-5A mix of peds / adults A decision is typically made within 7-14 daysAPCs with 3 years of experience in a similar setting PAs must have a CA Controlled Substance Education Course Certificate NPs must have a Furnishing License with Schedules IIActive DEA needed before the startCredentialing takes 30-90 daysThe following immunizations are required for credentialing (before the start date) : Flu Vaccine, TB Status / X-Ray Results (results must be within 90 days before the start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years). HCO & Vendor agree to a 30-day notice for ending all assignments.
• 1099 Pay Rate : Competitive Market Rates (W-2 Pay Packages available upon request)
• Certification Requirements : Please confirm credential requirements with VHS upon application.
