Job description

Basis Function

To provide daily transportation in order to support and meet the care delivered to patients / residents requiring long - term or rehabilitative care, in accordance with the established nursing care procedures and as directed by your supervisor.

CHARACTERISTIC DUTIES AND RESPONSIBILITIES

ESSENTIAL FUNCTIONS

  • Provide transportation and care to and from daily appointments to meet the needs of patients / residents
  • Assist with and / or transport residents / patients to recreational functions.
  • Assist with and / or transport specimen for Jabs.
  • Daily scheduling of transport and obtaining applicable i nformation from Tenn Care and / or appropriate financial resources.
  • Assure family / resident notification of transports .
  • Perform other related duties as assigned or requested .
  • Maintaining vehicle 's safe driving condition by assuring upkeep of maintenance and logging.
  • Assure equipment is in a safe and working order.

MINIMUM QUALIFICATIONS

  • Graduation from high school.
  • Good driving record.
  • Driver's license -preferred Chauffeur's (Endorsement For higher) but not required .

    • CNA a plus.
