About Facility : o 5003 Hospice Lane, Kannapolis, NC 28081 ? Would work remote (out of their home) to start the day and then would go into the field. ? Would only go into the office for biweekly IDG meetings (every Wednesday), orientation, training and to obtain supplies. ? General Information : o This is a field Hospice position requiring travel into patient homes and LTC facilities ? Will serve Assisted Living and SNF s as well as personal homes ? Areas served : may be subject to change, Charlotte metro area and Harrisburg (western corridor of Cabarrus County) o Hours of Operation : 0830 to 1700 ? Expectation is availability during those hours Mon-Fri, with the opportunity to pick up night / weekends if they choose o Minimum Years of Experience required : 1-2 years of Hospice experience in the home home visit experience is hard stop, please decline without this. o Will you accept a first-time traveler : Yes ? Patient Types / Job Duties : o End of Life and Hospice eligible patients o This is a FIELD position requiring traveler to commute and provide care within the home setting o Biweekly Case Conference Process : every other Wednesday there is an IDG meeting where the interdisciplinary team will discuss the patient load ? RN will do IDG notes showing continued eligibility and that the patient meets Hospice criteria ? Typically will take 1.5 hours in the home office will also see patients this day o Mileage Reimbursement : will go off current IRS rate - each day the phone will map and calculate the mileage which then will feed into another system and is processed on biweekly paycheck ? Visits per Day : o Territory is assigned by location and therefore the caseload can fluctuate depending on patient deaths. etc. o Average caseload has been 14-15 patients, doesn t always equate to the same of patients per day, ideally each RN would have 5-6 visits per day, but this will vary (25 per week is goal). o Traveler will have autonomy and will manage their own schedules. o Average drive time / day varies based on region served. ? Required License, National Certification, Certs (BLS, ACLS, etc.), or other position specific requirements such as Driver's License and / or Insurance : o RN : BLS; Driver's License and proof of vehicle insurance ? Skills required : o Experience with Homecare Homebase is required o Experience working Hospice Home Health, as well as visiting patients on hospice care in Long Term Care settings o Catheter insertion and care, feeding support to include g-tubes, CAD pumps for pain control, medication management, psychosocial support and strong listening / communication skills are needed in this role, assessment of patient / family needs, bereavement support, ability to teach families and patients on EOL and expected decline throughout the EOL process. ? Technology / Equipment : o EMR : Homecare Homebase EMR o Will be given a tablet and a laptop (not used in field used for home charting) ? Orientation / training through IT and Nurse Educator ? Floating : If yes, where? o In field position ? Orientation (facility and unit) : o Minimally, 1 week (shadowing / training with other care givers), dependent on skillset and comfort level ?
