  • Supportive, positive, and collaborative work culture\n\nPosition Summary\n\nThe Registered Nurse (RN) Case Manager coordinates and provides skilled nursing care for home health and hospice patients. This position emphasizes patient education, compassionate care, and coordination with the interdisciplinary team to ensure the best possible outcomes.\n\nResponsibilities\n\nPatient Care\n\n
  • Develop and implement individualized plans of care with patients, caregivers, and the care team.\n\n
  • Deliver skilled nursing care as ordered by the physician.\n\n
  • Educate and support patients and families on treatment and care plans.\n\n
  • Document visits, updates, and changes in patient condition.\n\n
  • Assist with discharge planning and continuity of care.\n\nCommunication & Coordination\n\n
  • Maintain regular communication with physicians and interdisciplinary team members.\n\n
  • Participate in case conferences and agency quality initiatives.\n\n
  • Coordinate necessary equipment and community resources.\n\nLeadership & Support\n\n
  • Supervise and evaluate Home Health Aides and LPNs / LVNs.\n\n
  • Participate in on-call rotation and staff training programs.\n\n
  • Contribute to continuous quality improvement efforts.\n\nQualifications\n\n
  • Current Registered Nurse (RN) license in the State of Utah\n\n
  • Graduate of an accredited nursing program\n\n
  • Minimum 2 years of nursing experience, preferably in home health or hospice\n\n
  • Current CPR certification\n\n
  • Valid driver's license and insured vehicle\n\n
  • Strong communication, assessment, and problem-solving skills\n\n
  • Self-directed, compassionate, and team-oriented
