Job Description :

We are seeking a Registered Nurse (RN) to take on a leadership role in our Skilled Nursing Unit as a Resident Case Manager (RCM) . You will ensure the highest quality of care for our residents in a supportive and innovative environment. Our community has strong occupancy, excellent survey results, and long-tenured staff, offering you a predictable and stable work schedule.

Key Responsibilities :

• Coordinate and supervise resident care from pre-admission through discharge, including health status, rehabilitation, and financial coverage.
• Lead the interdisciplinary team in the RAI process (MDS scheduling, completion, submission, and validation per State and Federal regulations).
• Develop Nursing Care Plans, set measurable goals, implement interventions, and evaluate outcomes.
• Perform ongoing case management from pre-admission authorization through discharge.
