• Monday - Friday, 8 am - 5 pm, full time
• 6 - 8 home visits daily plus 12 - 15 facility patients daily
• Home and facility-based skilled nursing care
• Mobile wound care and chronic care management
• One facility assigned per day
• Geriatric patient population
• We provide complimentary housing and travel
