What Youll Do

As an RN Case Manager, youll be the cornerstone of compassionate care, providing support and expertise to patients in their homes, assisted living facilities, or skilled nursing homes. Your role will include :

• Serving as a trusted source of comfort and guidance for patients and their families
• Educating families on disease progression, self-care, and nutrition
• Collaborating with an interdisciplinary hospice team to develop and implement personalized care plans
• Managing symptoms with expertise and advocating fiercely for patient comfort
• Maintaining accurate, timely clinical documentation to ensure seamless care
