Job description

Job Title : Nurse Practitioner

Location : 375th Medical Group, Scott Air Force Base, IL 62225

Second Location : O'Fallon Family Medicine Clinic, St. Elizabeth Hospital, O'Fallon, IL 62269

Work Schedule : Monday - Friday, 7 : 00 AM 5 : 00 PM (8-hour shifts with a 1-hour unpaid lunch, 40 hours per week)

Qualifications :

Education : Master's degree in Nursing. Graduate from a program accredited by the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE)

Experience :

  • Minimum of 2 years of recent clinical experience as a Nurse Practitioner in a primary care or family medicine setting.
  • Must meet all clinical competency and credentialing requirements per Defense Health Agency (DHA) Procedure Manual 6025.13, Volume 4 .
  • Experience providing comprehensive health assessments, diagnosis, and treatment of acute and chronic illnesses, and performing preventive health services.

License / Certification :

  • Active, full, and unrestricted Registered Nurse (RN) license in any U.S. jurisdiction.
  • Basic Life Support (BLS) certification from the American Heart Association.
  • DEA Registration Number
  • National certification as a Nurse Practitioner by ANCC or AANP or Other nationally recognized specialty certification boards (e.g., National Certification Corporation for OB / GYN and Neonatal specialties).
  • Other Requirements : Must be a US Citizen.

    Responsibilities :

  • Provide direct patient care including examination, diagnosis, treatment, and management of patients presenting with a variety of acute, chronic, and preventive health conditions.
  • Deliver care to ensure optimal patient outcomes and continuity of care.
  • Perform detailed patient assessments, obtain comprehensive health histories, and formulate differential diagnoses.
  • Order, interpret, and evaluate diagnostic tests, laboratory results to determine appropriate care & treatment plans.
  • Develop and implement individualized treatment plans based on evidence-based clinical guidelines.
  • Prescribe medications, therapies, and referrals as clinically appropriate in accordance with DEA registration and established privileges.
  • Conduct physical examinations, immunizations, annual health screenings, and wellness evaluations.
  • Promote preventive health practices through patient education and lifestyle counseling to support readiness and long-term wellness.
  • Educate patients and families on disease processes, treatment plans, medication management, and preventive health measures.
  • Counsel patients on nutrition, physical fitness, and healthy lifestyle behaviors.
  • Coordinate and communicate with physicians, nurses, specialists, and other healthcare professionals to ensure integrated and effective patient care.
  • Consult with supervising physicians for complex or emergent cases as required.
  • Accurately document all patient encounters, treatments, and progress notes in the electronic health record (EHR) in compliance with DoD and DHA documentation policies.
  • Ensure all documentation is complete, timely, and supports continuity of care.
  • Participate in quality improvement initiatives, peer reviews, and risk management activities to promote continuous improvement and patient safety.
  • Comply with all facility protocols, Joint Commission standards, and HIPAA regulations.
