About the Company
A long-established and highly reputable home health organization (operating successfully for 7+ years) is seeking an experienced Director of Nursing (DON) / Director of Patient Care Services (DPCS) to lead and oversee all clinical operations. This is a high-impact role responsible for ensuring clinical excellence, regulatory compliance, staff leadership, and exceptional patient care delivery across the agency.
About the Role
The Director of Nursing / Director of Patient Care Services is responsible for the overall direction, coordination, and supervision of home health clinical services. This role ensures compliance with Medicare Conditions of Participation, ACHC standards, and state / federal regulations while driving quality outcomes and supporting agency growth.
Responsibilities
- Clinical Leadership & Oversight
- Provide direction, supervision, and support to all clinical staff, including RNs, LVNs, therapists, CNAs / HHAs, and clinical supervisors.
- Ensure the delivery of high-quality patient care that meets clinical standards and regulatory requirements.
- Oversee patient care assignments, referrals, and care coordination within the agency.
- Lead the development, implementation, and evaluation of clinical goals, policies, and best practices.
- Regulatory & Compliance Management
- Ensure compliance with Medicare Conditions of Participation, ACHC standards, and all state / federal clinical regulations.
- Maintain oversight of clinical documentation, patient records, and quality reporting.
- Monitor census changes, workflow metrics, and operational indicators to proactively adjust staffing and resource allocation.
- Oversee QAPI activities, including performance improvement initiatives, clinical audits, and quarterly / annual reviews.
- Team Management & Development
- Directly supervise clinical supervisors and department leads; conduct performance evaluations and provide coaching.
- Manage clinical staffing, hiring, onboarding, and disciplinary actions when required.
- Lead ongoing education, in-service training, and competency programs for all clinical personnel.
- Maintain strong communication pathways across the interdisciplinary team.
- Operational Management
- Assist the Executive Director / Administrator in budget planning and operational decision-making.
- Ensure proper maintenance of clinical supplies, equipment, and inventory for daily operations.
- Support referral development and maintain strong relationships within the healthcare community.
- Serve as Acting Administrator when needed, with full authority.
Qualifications
Active RN license in California (required).
3–4 years of clinical management experience within a home health or related healthcare environment (home health is strongly preferred).1+ year in a supervisory or administrative capacity managing multidisciplinary teams.Required Skills
