License : State License Verification Required.

Must-Have :

• Completed prescreen must be attached upon submitting application.
• Resume and license verification also required.
• Covid test required 72 hours prior to start date with results issued 48 hours prior.
• Reference checks (3) needed.
• Criminal background checks (county and federal) for the last 7 years.
• Multi-State Criminal Search and Nationwide Sex Offender Search for the last 7 years.
• HHS / OIG and GSA searches.
• Social Security Number Trace required.
• 1 Step PPD or Quantiferon or proof of positive PPD and Chest X-Ray required.
• SAM search performed.

Description :

Responsible for providing physical therapy services to patients.

Assess and evaluate patient needs for physical therapy interventions.

Develop and implement individualized treatment plans.

Document patient progress and modify treatment plans as necessary.

Collaborate with other healthcare professionals to ensure comprehensive patient care.

Maintain a safe and clean working environment.

Participate in continuing education and professional development.
