Pay Range : $33 - $59 / hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities :

• Coordinate patient care : Help pediatric patients and families navigate their medical journey from appointments to follow-ups to community resources.
• Provide expert guidance : Educate families on care plans, treatment options, and available support services.
• Collaborate across teams : Work closely with physicians, specialists, and care managers to promote a seamless patient experience.
• Promote quality & safety : Ensure all care and documentation meet clinical and regulatory standards.
• Advocate for patients : Serve as a trusted resource, ensuring care is accessible, timely, and patient-centered.
• Drive excellence : Participate in process improvement, staff mentorship, and evidence-based practice initiatives.
