Case Manager Education Center

The Case Manager for the education center provides necessary supports and services that enables participants to succeed and advance within education, employment and career pathways; assume a lead role in implementing career plans and case management services; serve graduates in need of follow-up assistance and monitor retention services in order to meet all common performance measures stated by funding source with the focus being on helping young people achieve success, growth and professional development within vocations with career tracks. Position is :   Monday- Thursday 9am-2pm

RESPONSIBILITIES

• Maintain professional boundaries at all times.
• Work with the Program Manager, outreach worker, and the classroom teachers in the case management of a successful transition plan for each participant.
• Determine service needs, beginning with core services and barrier identification.
• Provide wrap-around counseling / case management services, as well as mentoring if needed to enable participant to fully utilize program services and to achieve their education and career goals.
• Assess participant’s career interests, abilities and values through testing and informal interviews.
• Work with graduating participants to assess individual needs for continuing support.  Together with the graduate, develop a transition plan to maintain contact and provide needed services after graduation.  Maintain detailed participant case files.
• Develop Career Cluster programming, career and labor market exploration activities and exercises, post-secondary tours.
• Develop career opportunities in the public and private sector for participants.
• Develop a career action plan with the participant , Program Manager and teachers.
• Assist participants in employment and / or post-secondary opportunities within chosen career path .
• Classroom and 1 : 1 instruction. Facilitate potential career tracks that match participant’s interest and skills, develop and practice habits and skills that will help participants be successful in their career. Help develop plan for pursuing participants chosen career and / or job that might include setting up interviews, completing job applications, job leads and / or applying for schools .
• Assess participant need for assistance with housing, legal, child-care and other independent living issues. Assist participants with problem solving.
• Assist participant to obtain driver’s license and / or develop alternative means of transportation to work and / or school .
• Assist participant in developing a budget and financial literacy skills .
• Become familiar with and provide follow-up requirements for various program grants and contracts.
• Document all contacts and effort to contact participants and graduates in files.
• Provide required participant and graduate information (employment status, type of employment, wage, school enrollment, etc.) for DataCCAB tracking .
• Coordinate ongoing services with graduates who are involved in both post-secondary education and employment pursuits.
• Follow up on past participants to provide support and obtain data on job acquisitions and retention rates.
• Trouble shoot any current job site problems to help graduate retain employment.
• Assist in a career development plan that will result in advancement within a workplace or within a career path.
• Provide follow-up assistance while a participant is in any type of training program or on a newly acquired job to assure retention and / or successful completion, mediate between employer and participant as needed .
• Work is performed in an office or classroom environment with the use of office equipment and by driving program participants to program related destinations and appointments.
• Attendance at internal and external meetings and trainings .
• Other responsibilities as assigned.
