Join our team in Thousand Oaks, CA, as a Home Health Physical Therapist for a 13

week contract, with potential extensions. Provide compassionate care to patients in their homes.

Responsibilities :

Deliver quality physical therapy in a home setting.

Conduct assessments and create care plans.

Use Home Care Home Base and OASIS for documentation.

Collaborate with healthcare professionals.

Qualifications :

Valid OTR / L license.

BLS certification.

2+ years of home health experience.

Experience with Home Care Home Base and OASIS preferred.
