Position Purpose :
Responsible for assisting with monitoring client progress and achievement of independent living processes while assisting with the coordination of the daily routine of care for residents.
Essential Duties and Responsibilities :
- Conducts regular apartment inspections and room searches in accordance with resident phase level and overall program concerns.
- Complete s assessment with residents each day and monitors their schedule to ensure they have productive plans for the day that include education, employment, and other productive activities.
- Facilitates orientation group for residents who are new or non-performing.
- Teaches residents to utilize adaptive, pro-social skills and responses through modeling and direct intervention.
- Provide s clients with the resources they need to reach personal goals.
- Monitor s client progress and advocate for changes to their treatment plans as needed.
- Ensures three planned recreational and / or cultural activities are scheduled each month, at a minimum.
- Conduct s daily apartment checks and facilities inspections as required, submitting work orders that identify maintenance issues and concerns.
- Ensures that apartments are appropriately furnished and ready with needed supplies for new and existing residents to the program.
- Perform s initial evaluations of client needs and develop s customized treatment plans to share with assigned Case Manager and Life Skills Specialist.
- Acts as a C ase A ide on cases requiring additional support by performing case management responsibilities and teaching hands-on life skills lessons (i.e. cooking sessions, cleaning sessions, opening bank account, resource development, public transportation training, etc.).
- Assists Case Manager and Life Skills Specialist in facilitating and transporting youth to needed appointments (i.e. medical appointments, education enrollment, benefits meetings, school, etc.) in accordance with the youth's treatment plan.
- Completes appropriate documentation including the program log, progress notes, and life skills verification forms, and thorough incident reporting in a timely manner.
- Distributes medication as needed and monitors medication compliance (prescription medications only). Documents medication compliance and / or non-compliance on Medication Administration Record. Notifies C ase M anager when refills are needed.
- Ensures cleanliness of the van by enforcing the program rules of van usage with residents (i.e. No smoking, no eating, and no drinking).
- Ensures van is taken for maintenance as required and recommended by the Vehicle Liaison and Program Director.
- Implements COMAR requirements and program policies and procedures and participates in all required COMAR trainings.
- Other duties as assigned.
Non-Essential Duties and Responsibilities :
Acts as liaison with outside agencies including schools, social service systems, judicial systems, and other organizations as necessary. Advocates for the needs of residents.Performs administrative or housekeeping tasks, as assigned.Retrieves, distributes and forwards program and resident mail.Other duties as assigned.Qualifications :
College degree (BA / BS) or high school diploma plus 2 years of experience.Dependable and reliable.Ability to work autonomously.Demonstrates cross-cultural competencies.Strong child care leadership with an ability to support and motivate a diverse staff serving vulnerable youth.Commitment to teamwork, collaboration, and the Agency's mission to progressively serve vulnerable families and youth.Must possess an attitude compatible with the goals and purposes of NCCF.Physical Demands / Work Environment :
While performing the responsibilities of the job, the employee is required to talk and hear. The employee is often required to sit and use their hands and fingers to handle or feel. The employee is occasionally required to stand, walk, reach with arms and hands, climb or balance, and stoop, kneel, crouch or crawl. Vision abilities required by this job require close vision working with a computer monitor. While performing the duties of this job, employees are occasionally exposed to moving mechanical parts and vehicles. The noise level in the work environment is usually quiet to moderate. While performing the responsibilities of the employee's job, these work environment characteristics are representative of the environment the employee will encounter. Salary : $19 - $20 / hour
