Responsibilities

This position performs health education work for the Healthy Start Program and the Okaloosa County Health Department (OCHD). Work is performed under the supervision of the Healthy Start Supervisor and the direction of Healthy Start Program Director, with adherence to all Florida Department of Health (FDOH), OCHD and Healthy Start policies and guidelines.

• Conducts case management and provides care coordination for Healthy Start clients of Okaloosa County, including women during pregnancy and to their infants after delivery.
• Provides health education and care coordination to clients and their families in a variety of settings and modalities (phone, video conferencing, in-office / clinic, home, work, and / or school).
• Records work-related mileage for a personal vehicle and maintains travel documentation; adheres to travel-related policies. Reimbursement is provided per DOHP 56-37-22 Travel Policy.
• Provides education in prenatal health, parenting, infant / child health; offers health education resources and referrals to other providers as indicated.
• Performs psychosocial assessments (e.g., ASQs, Domestic Violence Screenings) and makes appropriate referrals for high-risk clients.
• Develops individual client and family support plans to ensure appropriate services and minimize obstacles to prenatal and infant health care participation.
• Documents all client activities and codes services in the Well Families System with accuracy and timeliness; assists in data collection and evaluation to tailor education and resources to client needs.
• Participates in monitoring / QA activities, plans and participates in education / outreach activities, attends meetings and trainings, and joins committees / work groups as requested.
• Attends routine / episodic meetings with Supervisor / Program Director to review performance and development; attends Healthy Start Coalition meetings as requested; reports issues promptly.
• Performs other duties as assigned in support of the Agency's mission and vision.
