Travel Physical Therapist (PT) - $2,002 per week in Valley City, ND

Job Title : Physical Therapist Job Specialty : Home Health Physical Therapy Job Duration : 13 weeks Shift : Day Shift, Monday - Friday, 8 AM - 5 PM Guaranteed Hours : 40 hours per week Experience : Minimum of 1 year License : ND Physical Therapy License Certifications : BLS Must-Have : - Current Driver's License & Auto Insurance - PT-related skills & previous home health experience Job Description : - Provide home health physical therapy services to clients in a post-acute care setting, including those recovering from surgeries and falls. - Cover a 60-mile radius area, treating 5-7 patients per day. - Utilize EMR : HomeCare / HomeBase for documentation. - Conduct services independently, with no clinical involvement or manager on shift. - Orientation will range from 3-5 shifts. - Adhere to dress code : CSH-colored scrubs or business casual. - Manage care-setting : Home Health and Hospice patients. Note : No weekend or holiday shifts are required. Ability to self-schedule and block scheduling are not applicable.
