Job Description

The Outreach Worker for the Beach Cities Homeless Services Program will provide person-centered, relationship-driven support to individuals experiencing homelessness in El Segundo, Manhattan Beach, Hermosa Beach, and Redondo Beach. This multi-jurisdictional role strengthens coordination between outreach, case management, and city stakeholders to ensure participants receive the resources and advocacy needed to achieve housing stability and improved well-being.

The Outreach Worker engages individuals in the field, builds trust, assists with problem-solving and documentation readiness, connects them to behavioral health and supportive services, and facilitates pathways toward interim and permanent housing.

The Outreach Worker will be paired with a partner as often as possible to complete assigned tasks.

Essential Duties & Responsibilities :

Outreach & Engagement

  • Conduct proactive field outreach in encampments, parks, public spaces, and other non-traditional settings to locate and engage people experiencing homelessness (PEH).
  • Build rapport and establish trust through consistent, respectful, and non-judgmental engagement.
  • Provide immediate assistance such as food, water, hygiene kits, and referrals to emergency or interim housing resources.
  • Perform crisis intervention and de-escalation when needed to ensure client and community safety.

Case Management & Service Coordination

  • Conduct comprehensive assessments to identify each client’s strengths, needs, and barriers.
  • Develop individualized service and housing plans in collaboration with participants.
  • Connect clients to essential services including behavioral health, primary care, employment support, and benefits enrollment.
  • Support clients in obtaining identification, completing housing applications, and gathering necessary documentation for shelter or permanent housing.
  • Provide ongoing case management and advocacy to promote long-term housing retention and stability.
  • Conduct regular follow-ups and progress evaluations with clients; assist in goal adjustment as necessary.
  • Maintain active communication and coordination with city staff, local law enforcement partners, service providers, and community stakeholders.
  • Collaboration & Systems Coordination

  • Participate in multidisciplinary case conferencing meetings and interagency collaboration to coordinate care and identify housing opportunities.
  • Attend data sharing meetings, community engagement events, and city council meetings as assigned.
  • Collaborate with partner agencies including LA County Department of Mental Health, Department of Health Services, LAHSA, and other local nonprofits.
  • Ensure that service delivery aligns with trauma-informed, harm-reduction, and housing-first principles.
  • Data & Documentation

  • Maintain detailed, accurate records of all client interactions, services provided, and outcomes in designated systems (e.g., HMIS).
  • Prepare and submit timely monthly data and outcome reports in accordance with HERO and city requirements.
  • Uphold confidentiality and comply with all HIPAA, agency, and contract-specific data security standards.
  • Any and all other duties, tasks or responsibilities as requested by management.

    Skills, Qualifications & Competencies :

    Required Qualifications

  • Minimum 6 months of experience in homeless services, outreach, social services, or related field.
  • Strong interpersonal skills with the ability to build trust and maintain professional boundaries.
  • Demonstrated understanding of the Housing First model and harm reduction principles.
  • Basic written and verbal communication skills for effective engagement and documentation.
  • Proficiency in basic computer systems, including Microsoft Word, Outlook, and client management databases.
  • Ability to work both independently and collaboratively in diverse, outdoor, and fast-paced environments.
  • Culturally competent approach with sensitivity to individuals from varied backgrounds, life experiences, and identities.
  • Preferred Qualifications

  • Bilingual in English and Spanish .
  • Experience conducting field-based homeless outreach and housing navigation.
  • Familiarity with LAHSA, DMH, DHS, and other County systems of care.
  • Requirements :

  • Must be 18 years of age or older.
  • Must possess a valid California driver’s license and maintain a clean driving record.
  • Must have reliable transportation; occasional use of personal vehicle may be required (mileage reimbursed).
  • Must pass a Live Scan background check , drug screen, and criminal history clearance in accordance with applicable laws and program requirements.
  • Must provide proof of negative TB test (within the last 12 months) prior to hire and maintain current status thereafter.
  • Must be fully vaccinated for COVID-19 or provide valid exemption documentation consistent with applicable law.
  • Work Environment :

  • Work is primarily conducted outdoors in field-based settings with minimal office time.
  • Frequent travel within the Beach Cities area (El Segundo, Manhattan Beach, Hermosa Beach, Redondo Beach).
  • Requires walking, standing, and occasional lifting of up to 25 pounds.
