Licensed Practical Nurse / Per Diem

Unity Health and Rehabilitation Center is seeking Licensed Practical Nurses (LPNs) that truly love what they do and that care for our residents. We are looking for candidates that are friendly, fun, compassionate and most of all good listeners and team player

SHIFTS : PER DIEM - BOTH SHIFTS

PRIOR EXPERIENCE IN A SKILLED NURSING FACILITY PREFERRED

Bi-Lingual English / Spanish Required

Responsibilities of the Licensed Practical Nurse (LPN) :

• Provides plan care for all resident attending to.
• Documents the patient care activities performed on the unit.
• Must document changes and follow up on condition.
• Organizes, cleans, and maintains the patient / resident units.
• Ensures that resident's / patient's rights are adhered to.
• Participates in departmental activities and follows established guidelines.
• Performs related duties as required.
