Licensed Clinical Social Worker (CA, OR, WA)

A company is looking for a Licensed Clinical Social Worker (CA, OR, or WA license required).

Key Responsibilities

Provide ongoing support and education to individuals living with dementia and their families

Deliver a range of virtual services including psychosocial assessments and crisis support

Collaborate with an interdisciplinary team to ensure holistic and compassionate care

Required Qualifications

Active, unrestricted LCSW license in CA, WA, or OR, and residence in one of the states of operation

3+ years of experience in care management and behavioral health services for dementia or geriatric settings preferred

Experience providing virtual services is strongly preferred

Knowledge of evidence-based practices for the geriatric population

Strong organizational skills and proficiency in maintaining records and utilizing relevant software
