Looking for a qualified Physical Therapist. Assigned county Territory is Area 4 (Danville, Martinez, Moraga, Alamo, Clayton, Concord, Lafeyette, Orinda, Pleasant Hill, Walnut Creek) and Area 5 (Pittsburg, Discovery Bay, Antioch, Brentwood, Oakley, Bethal Island, Bay Point) Additional travel outside area may be required. Weekend coverage may be required. If Guarantee, the following shall apply : Productivity : At least 27 points per week (points described below). 36 hours Guarantee. Traveler must complete 30 points in the week. Must be willing to travel outside the area or work weekends if required to maintain weekly visit expectations. If Traveler refuses patients, guarantee does not apply. Guarantee does not apply for 1st and last week of assignment, or for weeks in which there are holidays, or for weeks in which the traveler has time off. If Non-Guarantee, the following shall apply : To secure a full caseload, Traveler may be required to go outside the service area occasionally. Can refuse patients. No guaranteed hours. Traveler must be willing to see Covid and Covid Pui patients. Payment made on per visit basis. See below for visit types and how they convert to points and how they convert to hours. Routine, Eval, Discharge, Eval SoC, Eval RoC, Eval ReCert, Eval F / up, Re-Eval, Eval Discharge, Oasis Discharge, Oasis F / up = 1 point = 1.33 hours. OASIS SoC and OASIS ReCertification and OASIS RoC = 1.5 point = 2 hours. Note that Rate is all inclusive and covers drive / travel time, visit time, documentation time, & time spent in making phone calls to patients & doctors .
