Top 10 Diagnosis / Procedures :

General outpatient ortho / neuro mix across the lifespan, spinal pain, post-op recovery, post-acute therapies.

Monday, Wednesday, Thursday, Friday 8 : 00 AM - 6 : 30 PM, 45-minute sessions.

Pelvic floor experience preferred but not required.

Most Common Equipment : All equipment provided.

Job Summary

Responsible for evaluating patients, determining plan-of-care and goals of treatment, and participating in patient treatment within the scope of Physical Therapy. May oversee therapy assistants and / or students in implementing treatment plans and provide supervision to therapy aides and / or volunteers. Works as part of a patient care team to achieve the best outcomes for the patient.

Essential Functions

Evaluates assigned patients and establishes treatment plans and goals using data from various sources, such as patient records, interviews, observations, and team input.

Provides treatment according to the plan of care, modifies plans to meet patient needs, and updates plans as appropriate.

Completes documentation accurately and timely, adhering to regulatory and state licensure requirements and professional standards.

Collaborates with other healthcare professionals to ensure smooth and continuous patient care. Acts as a liaison with nursing, medical, and therapy staff to facilitate problem-solving and service coordination.

May mentor and oversee new caregivers, physical therapist assistants, and / or therapy aides. Provides clinical instruction and training to students.

Recommends and participates in the development of new services and programs, quality improvement, and monitoring activities.

May participate in staff meetings, patient care meetings, educational in-services, caregiver orientation, and other professional activities.

Performs other duties as assigned.

Experience :

1 year of EPIC experience preferred.

Credentials :

Required : Physical Therapist - Oregon.

Required Upon Hire : Basic Life Support.
