JOB SUMMARY
The primary role of the Licensed Substance Abuse Professional is to assist in the delivery of this 24/7/365 service, utilizing a team approach, including the provision of services that will include direct and indirect interventions. CST is provided in a variety of locations. The team community-based mental health and substance abuse rehabilitative services and support through a team approach necessary to assist adults in achieving rehabilitative and recovery goals.
Community Support Team (CST) provides direct support to adults with a Diagnostic and Statistical Manual of Mental Disorders Fifth Edition (DSM-5) diagnosis of mental illness, substance use, or comorbid disorder and who have complex and extensive treatment needs. This service consists of community-based mental health and substance use services, and structured rehabilitative interventions intended to increase and restore a beneficiarys ability to live successfully in the community. The team approach involves structured, face-to-face therapeutic interventions that assist in re-establishing the beneficiarys community roles related to the following life domains: emotional, behavioral, social, safety, housing, medical and health, educational, vocational, and legal.

General Role Requirements:
This is an intensive community-based rehabilitation team service that provides direct treatment and restorative interventions as well as case management. CST is designed to provide:
a. symptom stability by reducing presenting psychiatric or substance use disorder symptoms;
b. restorative interventions for the development of interpersonal, community, coping, and independent living skills;
c. psychoeducation;
d. first responder intervention to de-escalate a crisis; and
e. service coordination and ensure linkage to community services and resources.
This team service consists of a variety of interventions available 24 hours a day, 7 days a week, 365 days a year, and delivered by the CST staff, who maintain contact and intervene as one organizational unit. CST services are provided through a team approach, however discrete interventions may be delivered by any one or more team members if clinically indicated. Not all team members are required to provide direct intervention to each beneficiary on the caseload.

Supervision Requirements:
Clinical supervision for the CST Staff is provided by the licensed Team Lead who has the knowledge, skills, and abilities required by the population served. Supervision is provided and individualized to all team members by the CST Team leader, according to supervision requirements specified in 10A NCAC 27G.0104. Licensed SA Professionals are required to participate in supervision as outlined in the individualized supervision plan.

JOB DUTIES AND RESPONSIBILITIES
  • Provides psycho-education as indicated in the Person-Centered Plan.
  • Assists with crisis interventions
  • Assists the Team Lead with behavioral and substance use disorder treatment interventions.
  • Assists with the development of relapse prevention and disease management strategies.
  • Participates in the initial development, implementation, and ongoing revision of the PCP.
  • Communicates the beneficiarys progress and the effectiveness of the strategies and interventions to the Team Lead as outlined in the PCP.
  • Provides intensive case management.
  • Linkage and referral to formal and informal supports.
  • Monitoring and follow-up.
  • Completes functional needs assessment(s) to determine the scope and anticipated outcomes of the services.
  • Assist with beneficiary housing search including engaging landlords to rent to beneficiaries and writing reasonable accommodation letters.
  • Assist with connecting beneficiaries to financial and in-kind resources to set up and maintain their household.
  • Prevent and mitigate housing crises including being a point of contact for landlord concerns.
  • Assist with rehousing beneficiaries if they are no longer able to stay in their unit due to eviction or risk of eviction.
  • Assist in developing daily living skills to stabilize and maintain housing.
  • Linkage to the most clinically appropriate and effective services including arranging for psychological and
psychiatric services
  • Coordinates time at the location where services are being performed as specified in the service
definition
  • Provides staff with individual-specific training and training in the knowledge, skills and abilities required
by the population and age to be served (adults 18 and older with MH/SA issues).
  • Participates in a first responder on-call system available to consumers and/or his/her natural support
network on a 24/7/365 basis; coordinates first response resources according to consumer needs and
the PCP.
  • Consult with identified medical (for example, primary care and psychiatric) and non-medical providers,
engages the community and natural supports, and includes their input in the person-centered planning
  • Monitors and documents the status of the recipients progress and the effectiveness of the strategies
and interventions outlined in the Person Centered Plan
  • Coordinates transition to another level/type of care for the person supported
  • Coordinates and oversees the discharge planning process including the development of a discharge plan initially upon admission and a discharge summary with follow-up resources after services
  • Practice standard medical precautions by understanding and utilizing personal protective and safety equipment
  • Ensures confidentiality regarding sensitive and protected information
  • Ensures individual rights to privacy and protected health information for the person supported
  • Maintaining records and documenting each individual and reporting unusual and critical incidents in a professional, timely manner (within 24 hours)
  • Familiarization with medications used by the client and policies regarding medication administration, as applicable.
  • Performs all other duties as reasonably required and assigned
QUALIFICATIONS
One FTE dedicated team member who is a licensed substance abuse professional. A team member can be a Certified Clinical Supervisor (CCS), Licensed Clinical Addiction Specialist (LCAS or LCAS-A), Certified Substance Abuse Counselor (CSAC), or a Certified Alcohol and Drug Counselor (CADC).
  • "Certified clinical supervisor (CCS)" means an individual who is certified as such by the North Carolina Substance Abuse Professional Practice Board.
  • "Licensed clinical addictions specialist (LCAS)" means an individual who is licensed as such by the North Carolina Substance Abuse Professional Practice Board.
  • "Certified substance abuse counselor (CSAC)" means an individual who is certified as such by the North Carolina Substance Abuse Professional Certification Board. (To comply with NC General Assembly Session Law 2019-240 Senate Bill 537, the licensure name for The certification name for Certified Substance Abuse Counselor (CSAC) is amended to Certified Alcohol and Drug Counselor (CADC). Policy amendment(s) will be effective the date the related rule for 10A NCAC 27G is finalized).
Education/ Certification/ Skills
  • The licensed substance abuse professional shall meet the requirements specified for the Certified Clinical Supervisor (CCS), Licensed Clinical Addiction Specialist (LCAS or LCAS-A), Certified Substance Abuse Counselor (CSAC), or Certified Alcohol and Drug Counselor (CADC) according to 10A NCAC 27 G. 0104 and have the abilities, skills, and knowledge required by the population and age to be served.
  • Must have related educational and professional experience to meet licensed/certified professional status/credential;
  • It is expected that licensed (or provisionally licensed, under supervision), certified staff will practice within their scope of practice;
  • Must have a minimum of one (1) year documented prior experience working with adults with MH/SA/DD diagnosis.
  • Must maintain strict confidentiality;
  • Must possess effective communication/documentation skills and a valid drivers NC license;
  • A criminal history check is required;
  • Must agree to random drug screening
Work Environment
Work is performed primarily in the clients living environment and the community and may involve exposure to cigarette smoke, domestic animals/pets, household pests, uncomfortable heating/cooling, and other issues related to the domestic location in which service is delivered. OTHER REQUIREMENTS: Must be able to write legibly and/or possess basic word processing skills, or be willing to learn. Documentation requirements consist of a full-service note for each contact or intervention. Requires flexibility in scheduling and availability to adequately meet the service needs of the consumer/family and other team members. Must have reliable transportation and be willing to travel locally.
