American Traveler is seeking an experienced Home Health RN Case Manager in Vermont with OASIS and case management experience.

\n \n Responsibilities\n

  • Home health setting providing patient care in the community
  • Day shift schedule, Monday through Friday with weekend rotation
  • Position is based in Vermont
  • Role includes case management responsibilities
  • Preference given to candidates with prior travel nursing experience
  • No requested time off accepted for the duration of the contract
  • Local candidates are accepted
  • Must be available for weekend rotation as scheduled

  • Active VT or compact RN license
  • Minimum 1 year of home health nursing experience
  • OASIS experience required
  • Home health and case management experience required
  • BLS or BCLS certification required
  • Active license must be currently in hand
