Bereavement Counselor
Day, 8 : 30am - 5pm
May be required to work 1-2 evenings a week
Very occasional weekends required
ESSENTIAL FUNCTIONS :
Assists with review of all referrals, distribution of initial bereavement materials, and scheduling contacts.
Assesses all bereaved to determine appropriate bereavement support
Makes home visits as needed.
Addresses high-risk needs and assures timely ongoing assessment of bereaved individual's needs.
Assesses needs of grieving children, adolescents and / or families when requested.
Plans and facilitates support groups.
Available for counseling needs in times of crisis to both hospice families as well as community bereaved
Participates in planning and implementation of commemorative and other special events
Responds to community inquiries and requests for education and support.
Serves as a clinical resource to bereavement volunteers, oversees their orientation and training activities in bereavement programs.
Attends weekly meetings and offers anticipatory grief and bereavement resources, education, and consultation to team members as well as patients and families
Contributes statistical data for timely and accurate reports reflecting departmental activities
Assures timely, accurate and appropriate documentation of bereavement contacts.
Help to organize and attend a weekend children's bereavement camp
QUALIFICATIONS :
Bachelors Degree in a human services related field required
Two years relevant experience required.
Able to listen, assess needs, and counsel bereaved persons individually and in group settings.
Able to communicate effectively, build positive relationships and demonstrate confidence with business and community professionals.
Ability to use computer; basic understanding of Microsoft Office required
Must be able to move about freely in all types of settings, all of which may not be required to be accessible.
