Job description

A company is looking for an LPN Health Coach in Care Management.

Key Responsibilities

Provide support to patients for clinical outcome management and healthy lifestyle information

Complete transitions of care calls for moderate risk patients and arrange follow-up services

Assist in patient instruction and health literacy, monitoring outcomes and documenting in patient records

Required Qualifications

Certificate Program Post High School Vocational / Specialized Training Required

1 year of healthcare experience required

Licensed Practical Nurse or Multi-State License upon hire required

Basic Life Support certification upon hire required

Completion of required education programs and competency testing upon hire required
