Job Details

Deliver direct nursing care in a fast-paced primary care setting

Educate patients on chronic disease management, medication adherence, and wellness practices

Assist in coordinating care and managing patient follow-ups

Perform triage, health assessments, and routine procedures

Maintain accurate documentation in electronic medical records (EMR)

Support care teams in quality improvement and patient-centered initiatives
