Local Contract Nurse RN - Home Health - $65-70 per hour

IMCS GROUP is seeking a local contract nurse RN Home Health for a local contract nursing job in Anaheim, California. Job Description & Requirements Specialty : Home Health Discipline : RN Start Date : 10 / 20 / 2025 Duration : 13 weeks 40 hours per week Shift : 8 hours, days Employment Type : Local Contract Seeking an experienced Registered Nurse (RN) for a Home Health assignment based in Anaheim, CA. The ideal candidate is a self-motivated, compassionate nurse with strong clinical skills and a passion for providing quality home-based care to a geriatric population. Schedule & Territory :

• Shift : Monday – Friday, 8 : 00 AM – 5 : 00 PM
• On-call / Weekend Requirements : None
• Coverage Area : Anaheim Hills, Anaheim, Orange, Villa Park, Yorba Linda
• Radius : Occasionally up to 50 miles; routes are generally well-grouped to reduce drive time Responsibilities :
• Deliver skilled nursing care in a home health setting
• Complete Start of Care (SOC), Resumptions of Care (ROC), Recertifications, and Discharges
• Accurately document patient information using HCHB EMR system
• Monitor and manage individualized care plans
• Coordinate with interdisciplinary team members
• Ensure high-quality care and patient satisfaction Productivity Standards :
• Daily Requirement : 6 points o 2 points : SOCs, Recerts, ROCs, Discharges (OASIS visits) o 1 point : Routine visits
• Patient Load : Up to 6 patients per day (30 points / week standard) Requirements :
• Experience : o Minimum 1 year of recent (within last 3 years) Skilled Home Health experience o Strong experience with OASIS documentation required
• Licenses / Certifications : o Valid California RN license o BCLS (Basic Life Support) certification
• Other Requirements : o Must provide own reliable transportation o Comfortable with travel within the assigned territory o Proficiency with HCHB (Homecare Homebase) EMR system required Proficiency with HCHB (Homecare Homebase) EMR system required Home Health experience Strong experience with OASIS documentation required Valid California RN license
