This is a field-based position requiring regular travel to various locations as part of your daily responsibilities. As part of our Senior Community Care (SCC) product, you will work collaboratively with a team dedicated to providing care to patients at home, in nursing homes, and assisted living facilities. This vital role enhances access to care and improves patients' health journeys.
You will serve as a key member of the primary care team, collaborating with all interdisciplinary team members while supporting multiple providers. The Care Coordination Specialist assists nurse practitioners (NPs) and physician assistants (PAs) in care coordination, administrative tasks, and supporting our patients.

This is a full-time position (40 hours / week) working Monday to Friday. Employees must have the flexibility to work various 8-hour shift schedules between 8 am and 5 pm (no nights, weekends, or holidays). Occasional overtime may be necessary based on business needs.
We provide two months of paid training, with training hours discussed on your first day of employment.
