We're hiring for Primary Care Chronic Care Managers.

EDUCATION/EXPERIENCE REQUIRED:

QUALIFICATIONS:

  • Active LPN license (compact)
  • Two (2) years of prior long-term care service coordination, care delivery monitoring and care management experience. This experience may be concurrent with the two years of experience working with geriatric population described above
  • Must reside in North Carolina or South Carolina
  • Must have ability to travel regularly as needed to perform job duties

EDUCATION/EXPERIENCE PREFERRED:

Experience working with member/recipients with co-occurring physical health and/or behavioral health needs preferred

WORK ENVIRONMENT/TRAVEL
Hours of operation are Monday-Friday 8am-5pm. Standard office equipment is provided and required to perform job duties. This is a hybrid position, that requires the candidate to have a private, quite working space, away from distractions, that is HIPPA compliant, while also requiring occasional onsite travel, to regional facilities.

LifeSource is looking for a customer service focused Chronic Care Manager to join our dynamic Primary Care Team! This role is crucial to LifeSource’s commitment to provide quality healthcare and better quality of life for the patients we serve. This person will be vital to ensuring our providers are able to serve each patient, that is needing our services, by working with the onsite provider both onsite as well as in a remote setting. DUTIES/RESPONSIBILITIES

The following duties and responsibilities generally reflect the expectations of this position but are not intended to be all inclusive, other duties may be assigned:

  • Work with both new and established patients to complete necessary discussions with patients and families on the benefits of chronic care management and obtain enrollment.
  • Complete Health Risk Assessments and determine RSCM scores, as deemed necessary
  • Coordinate and provide chronic care management discussions that are safe, timely, effective, efficient, equitable, and client centered
  • Handle case assignments, draft chronic care service plans and review case progress with health care providers.
  • Facilitate multiple care aspects (case coordination, information sharing, etc.) to include areas such as referrals, imaging, labs, and medication refills and adherence.
  • Develop effective working relations and cooperate with the medical team throughout the entire case management process.
  • Record cases information, complete accurately all necessary documentation and produce statistical reports.
  • Help clients achieve wellness and autonomy.
  • Coordinate with health care service providers, long term care communities, and clinical organizers as necessary.
  • Interact with patients to keep track of their progress and to ensure satisfaction.
  • Promote quality and cost-effective interventions and outcomes.
  • Assess and address motivational and psychosocial issues
  • Adhere to professional standards as outlined by protocols, rules and regulations.

SKILLS AND QUALIFICATIONS

  • Customer service/phone experience.
  • Possess the ability to multitask and stay organized.
  • Reliable and dependable.
  • Experience with electronic health record systems.
  • Prior health care scheduling or medical related experience.
  • General Knowledge of HIPAA privacy laws and medical terminology.
  • Strong computer skills with knowledge of Microsoft Word, Excel, Outlook, and general Microsoft (PC) and Apple Based products (iPhone, iPad).
  • Possess the drive to help patients and staff with tact and diplomacy.
  • Ability to communicate clearly and concisely in verbal and written form.
  • Strong communication skills and customer service attitude required.
  • Reliable vehicle

Must be able to frequently communicate with facilities and patients and/or with others about patient or provider requirement, complaints, and common inquiries.
