Pay Rate : $1,800.00 - $2,000.00 Gross Per Week

Number of Positions : 1

Start Date : 11 / 17 / 2025

Length : 13 weeks

Schedule : Monday - Friday 8 : 00 am - 4 : 30 pm

  • Weekends as needed
  • Mileage can be reimbursed

Additional Details :

  • Develops and revises plan of treatments and discharge plan in conjunction with patient, family, physician, care delivery team and third party payors including obtaining medical orders and approval of physical therapy services.
  • Consults with and educates the patient, family and other team members regarding disease process, self care techniques and preventive strategies. Implements home therapy programs including but not limited to : gait training, transfers, ADL's, home modification, care, and use of equipment.

    • EXPERIENCE REQUIREMENTS :

  • A B.S., M.S. or D.P.T. degree in Physical Therapy
  • Minimum of one year experience in Long Term Care / Home Care setting as Physical Therapy
  • Recent Home Care experience (within the last year)

    • CERTIFICATIONS REQUIREMENTS :

  • BLS

    • LICENSE REQUIREMENTS :

  • New York
