Responsibilities

• Assist Physical Therapists in developing and implementing individualized care plans for patients.
• Provide therapeutic interventions as directed by the supervising Physical Therapist.
• Monitor patient progress and report any changes or concerns to the supervising therapist.
• Participate in discharge planning and prepare patients for transition out of therapy services.
• Maintain accurate documentation of patient care and treatment sessions.
• Educate patients and their families on exercises, techniques, and home care strategies.
• Ensure a safe and clean environment for patient care by adhering to facility protocols.
