Physical Therapist

Our Home Health services focus on the rehabilitation and restoration of independence after surgery, an acute illness, as well as management and stabilization of chronic diseases. Be part of a skilled team who assist patients, families, and caregivers in the comfort and familiarity of their own home.

Under a plan of treatment approved by a physician, assists the patient in attaining maximum potential through application of physical therapy principles. Provides consultation to other health team members. Consults with patient's family and caregivers. Evaluates home environment and makes recommendations.

Status : Full-Time

Education / Experience / Skills / Qualifications : Currently licensed by the Physical Therapy Examining Committee of the California Board of Medical Quality Assurance. Minimum of one year of experience as a physical therapist, preferably in an acute rehabilitation setting. Familiarity with HMOs, managed care payor sources preferred.
