Texas Licensed RN Case Manager

A company is looking for a Case Manager RN.

Key Responsibilities

Assess member health status and coordinate care needs, including inpatient review and discharge planning

Develop and implement treatment plans while monitoring the effectiveness and making necessary adjustments

Identify and coordinate services and resources to promote optimal health outcomes for members

Required Qualifications

5+ years of related work experience in clinical nursing and patient assessment

Bachelor's degree or equivalent related work experience

Graduate of an accredited school of nursing

Registered Nurse (RN) state licensure and compact state licensure in good standing

Knowledge of medical terminology and cost containment practices in healthcare
