Job description

Title : Nurse - RN

FLSA Status : Full-time, Exempt

Location : West St. Paul, MN

Purpose :

The Registered Nurse is responsible for providing a full range of nursing services and oversees the health management of all individuals served in a residential setting for individuals who have a serious mental illness and / or a substance use disorder. This position assists with the admission process including screening referrals, completing assessments, and helping the team determine immediate and ongoing recipient needs.

Key Responsibilities :

Duties and Responsibilities :

General Nursing Services

  • Educate coworkers in the areas of health issues and medications. This includes training new staff and meeting with current staff as needed to provide reminders / coaching around topics of monitoring meds, documenting doses taken in EMAR, etc. This also includes providing education to the team at monthly all-staff meetings.
  • Educate recipients in the areas of health issues and medications. This includes facilitating educational groups and meeting with recipients individually.
  • Teach recipients to advocate on their own behalf, including in regard to their own rights.
  • Coach, model, or facilitate the skills necessary for recipient to meet their daily needs.
  • Coordinate mental health, chemical health and medical services.
  • Educate recipients to independently, and safely, manage their medications in preparation for living in the community.
  • Assist the team in screening referrals for appropriateness by reviewing referral forms and collateral medical records and / or participating in interviews.
  • Alert the team about the medical needs for specific clients, including potential side effects and / or symptomology.
  • Complete health assessments for all recipients within 72 hours of their admission. Include identified nursing recommendations for staff to reference as ITPs are developed.
  • Review recipient medications upon admission and transcribe those into EMAR immediately or before the end of the shift.
  • Work with recipients and the rest of the team to ensure that necessary medical appointments are scheduled.
  • Accompany recipients to outside appointments to coordinate with providers and ensure effective communication about that recipient's health and needs.
  • Coordinate with external providers ongoing regarding medications, symptoms and needs via phone calls and communication transfer forms.
  • Review doctor orders for medications upon admission and ongoing.
  • On an ongoing basis update EMAR to reflect medication changes and highlight those changes to the team as needed.
  • Assess individuals' to independently manage self-care and ability to follow through with medical recommendations and share with team.
  • Stay abreast of relevant health care issues and trends.
  • Participate in the on-call rotation, which includes responding to calls within 15 minutes, completing health assessments as needed, and coming on-site to assess recipient needs within 2 hours if situations arise that require it.

Crisis Assessment and Intervention Responsibilities

  • Recommend specific interventions if concerns about safety arise during the completion of a health assessment. Document these on the health assessment. Consult with the Clinical Supervisor if needed.
  • Observe and identify early distress signals and talk with staff about adjusting treatment interventions to prevent escalation.
  • Work with recipients and the rest of the team to identify what stressors may lead to crisis situations for specific recipients, as well as their early warning signs to address and mitigate future crises.
  • Promptly communicate with the rest of the team if a crisis has occurred, including what action was taken and what the plan is.
  • Assess recipient medically and assist with medical interventions as needed.
  • Completed necessary documentation if crisis situations occur, including an Incident Report, Service Delivery, Medication Incident Report. Communicate situation to individual's doctor if needed.
  • Required Skills / Abilities :

  • Knowledge of mental illness and the needs of people with mental illness.
  • Knowledge of substance use disorders and the needs of people with substance use disorders.
  • Knowledge of a wide range of medications, particularly psychotropics.
  • Ability to assess a wide range of residents' needs.
  • Ability to provide education on topics related to health (including medications) to individuals and groups, including individuals served and coworkers.
  • Ability to develop and maintain receptive, open, and approachable lines of communication with clients and staff, and provide a friendly environment for visitors.
  • The ability to provide on-call nursing services, which includes : responding to calls within 15 minutes, completing crisis health assessments (as needed) within 24 hours of admission, completing health assessments for IRTS (as needed) within 72 hours of admission, and coming on-site to assess recipient needs within 2 hours if situations arise that require it
  • Ability to use medical equipment such as blood pressure cuffs, scales, stethoscopes, glucometers, pulse oximeters, thermometers, needles and syringes
  • Ability to read Mantoux tests and document results
  • Ability to assess medical issues and implement the care needed, including delegation to higher levels of care if needed such as summoning paramedics or getting recipients to an ER or hospital
  • Strong written and verbal communication skills, including composing, proofreading, and editing.
  • Presentation and meeting facilitation skills.
  • Ability to work as a proactive and positive member of a team to deliver quality services.
  • Ability to maintain confidentiality of organization, personnel information, and client data.
  • Ability to use sound judgment and maintain healthy professional boundaries.
  • Analytical and problem-solving skills.
  • Ability to meet deadlines and set priorities.
  • Proficiency with a variety of web-based software and databases, including Microsoft Office Suite.
  • Working Conditions :

  • This position is based in the Dakota, Ramsey, or Scott County program office and will have interactions with clients of the program and the public.
  • Some activities may be required in the community.
  • Travel may be required to other program sites.
  • This position has the potential to come into contact with bodily fluids, communicable diseases, and tobacco smoke.
  • Prolonged periods at a desk, working on a computer.
  • Manual dexterity for tasks such as taking notes, using technological devices for documentation, or assisting clients with certain activities if needed.
  • Must be ambulatory and able to negotiate stairways.
  • The ability to lift and carry heavy weights that could be up to 25 pounds.
  • The ability to provide on-call nursing services.
  • Education and Experience :

  • Bachelor's degree in nursing (BSN).
  • Licensed Registered Nurse (RN) in Minnesota.
  • 3 years of previous professional experience providing nursing care to people with serious and persistent mental illnesses.
  • Additional Requirements :

  • Must not be actively receiving services from Guild and must not have received services within the last two years.
  • A valid Class 'D' Driver's License, reliable vehicle, satisfactory driving record and necessary insurance.
  • Current CPR / First Aid Certification is required.

    The salary range for this position is between $80,000-$85,500 annually. The range reflects the minimum and maximum salary for the position. Compensation will depend on the candidate's professional experience, skills, and relevant education, training, and certifications.
