Requirements / Qualifications

• Certification (Copy of Valid Vocational Nurse License)
• CPR / First Aid Certification (CPR Certificate and First Aid Certificate)
• Other (Valid Driver's License and ability to provide own transportation required - please DO NOT ATTACH license copy to application)
• Proof of HS Graduation (Acceptable formats : high school diploma or equivalent; transcripts [must show name and graduation date].
