The Role
The Hospice Medical Social Worker (MSW) is a dedicated professional who, in accordance with the treatment plan, supports the physician and other team members by addressing the social and emotional factors related to health challenges.
Key Responsibilities :
- Provide social casework to individuals and families receiving nursing or related services from the Agency.
- Assess families' financial situations as appropriate, considering the patient's prognosis and medical needs, while facilitating referrals to financial assistance as needed.
- Respond to referrals for casework from Agency staff or outside professionals, delivering services as suitable.
- Participate in interdisciplinary team conferences to identify and discuss social issues, their impact on medical situations, and consider alternate solutions.
- Refer patients and families to community agencies, ensuring appropriate follow-up.
- Engage in case conferences with other agencies to coordinate care.
- Educate staff about social resources and health services available through special agencies.
- Ensure comprehension and application of the 4Ms (What Matters, Medications, Mentation, Mobility) while delivering Age-Friendly Care.
Skills and Experience Required :
Master's degree (MSW) from a CSWE-accredited School of Social Work.Current Social Worker License in the state of practice in accordance with state requirements.Valid driver's license with a good driving record.Strong clinical skills and judgment.Ability to assess and document patient needs accurately, formulating tailored care plans.Excellent verbal and written communication skills.Proficiency in personal computing, including email, clinical software, and spreadsheets.Preferred Knowledge, Skills and Experience :
Advanced Palliative Hospice Social Worker Certification (APHSW-C) from the Social Work and Palliative Care Network.Two years of experience in hospice settings.Certification in Volunteer Administration (CVA) from the Council for Certification in Volunteer Administration.Experience with Electronic Medical Records (EMR).
