Overview : The Home Health RN will be handling roughly 30 patient cases where they will travel to patient homes (5-7 per day) to do in home assessments, wound care, medication administration and refills, patient education and helping to create a nursing care plan.
Responsibilities :
- Completes initial evaluation visits, including history and physical exam.
- Regularly evaluates the patient's nursing needs.
- Develops nursing care plan and makes necessary revisions.
- Provides health services requiring substantial specialized nursing skills including appropriate preventative and rehabilitative nursing procedures.
- Utilizes other community agencies and resources to provide comprehensive patient care.
- Coordinates additional health services provided to patient.
- Discuss changes in the patients' condition and needs with the physician and other appropriate personnel.
- Supports the patients' family in meeting the physical, psychosocial and other health needs of the patient.
- Oversees work of Home Health Aides as required in the patient’s home.
- Provides community consultation and health education when appropriate.
- Keeps accurate records of patient’s progress.
- Has overall responsibility for maintaining their patient’s records according to Home Health Services’ protocol.
- Participates in the regular case review meetings.
- Participates in the weekend / holiday rotation.
- Must provide a minimum of 25-30 patient encounters per week for a full-time position.

Monday-Friday30-40 hours / week (ideally 40 but can be a bit flexible)Can do 8 or 10 hour shifts between 8am-6pmThey will assign an area to the RN but they serve patients from East Boston down to Mattapan (all of Boston)
