Key Responsibilities
Member Support & Navigation
- Build trusting relationships with members and meet them “where they are.”
- Assess oral health needs, social determinants of health, and barriers to care.
- Support members in following provider treatment plans and home care instructions.
- Assist members in locating dental providers, scheduling appointments, and arranging transportation or translation services.
- Help members understand available dental benefits.
- Address barriers such as dental phobia, behavioral health concerns, or lack of knowledge about oral health.
- Provide in-person support when appropriate.
- Collaborate with referral sources (health systems, state agencies, community organizations) to co-manage oral, medical, and behavioral health needs.
- Refer members to social services to reduce external barriers to care.
- Document all actions in the care coordination platform according to program standards.

Bachelor’s degree in social work, human services, community health, or a related field (preferred).Experience in care coordination, case management, or community health navigation (health systems, Medicaid programs, or behavioral health preferred).Experience supporting populations facing barriers to care.Bilingual skills strongly preferred.
