California Licensed Care Coordinator

A company is looking for a Care Coordinator to facilitate client care and manage referrals within behavioral healthcare settings.

Key Responsibilities

Serve as the primary contact for partnerships requiring concierge care or closed-loop referral follow-up

Track, manage, and document the lifecycle of referrals for Intensive Treatment Services (IOP and PHP)

Coordinate with internal teams and external organizations to ensure timely placement and enhance care continuity

Required Qualifications

High school diploma or equivalent required; Associate's degree in a healthcare-related field preferred

Minimum 2-3 years of experience in care coordination or referral management within behavioral healthcare

Familiarity with higher levels of care (IOP, PHP, inpatient) and behavioral health systems

Experience with electronic health records and case tracking systems

Experience working remotely and / or with virtual care models is a plus
