DESCRIPTION OF WORK
As a Social Worker 1, you will provide a range of professional social work services, support, and interventions for residents of the facility. You will interact with residents, residents' representatives, and clients to coordinate needed social services. As a member of an interdisciplinary care team you will complete psychosocial assessments, provide direct social work practice, counsel residents and their family members, and prepare residents for discharge. This position requires excellent relationship-building skills and a steady composure to maintain positive and constructive working relationships with care teams, while responding to the needs and sensitivities of residents. In addition, you will participate in staff training programs, follow established procedures, and recommend quality improvement practice adjustments that enhance resident care.
