Certified Nursing Assistants (CNA) at St. Mary Home Care and Hospice

St. Mary Home Care and Hospice, a member of Trinity Health at Home, is seeking compassionate Certified Nursing Assistants (CNA) looking to make a meaningful impact in their communities. We're seeking motivated CNAs to serve patients in the greater Philadelphia 5-county area, providing comfort and support directly to patients in their homes. $3K Sign On Bonus offered. Signing Bonus is not applicable to internal employees, former employees who have resigned in the last year or applicants sourced from outside firms.

Position Purpose : Our hospice / home health aides provide paraprofessional services to our patients in their homes, following physician orders and in compliance with the state's Nurse Practice Act, any applicable licensure / certification requirements, and our policies and procedures.

What You Will Do :

• Follows care plan developed by RN.
• Reports client changes in condition to RN, as described on care plan.
• Assists clients with personal care activities including bathing, mouth care, denture care, skin care, grooming, bathroom privileges, transferring, ambulation, and dressing and undressing activities.
• Assists clients with household tasks including planning and preparing meals, feeding, light housekeeping, changing linens, making bed, and laundering clothes and bedding.
• Measures and records client's oral, rectal and axillary temperatures, pulse respiration and blood pressure as ordered.
• Performs range of motion and other simple procedures as an extension of therapy services as assigned.
• Assists clients in self-administration of medication. This procedure must be assigned by an RN and documented in the clinical record. The Home Health / Hospice Aide shall not administer medication of any kind.
