Diagnoses include : prematurity, cerebral palsy, Down syndrome, autism spectrum disorder, sensory processing disorder, oral aversion, ADHD, coordination impairment, concussion Special Procedures / Unit Details : outpatient multidiscipline (PT / OT / SLP) pediatric unitthe clinic is open mon - thurs 730-6 and fri 730-430 - specific days and hours will be dictated by skill set of applicant Special Equipment : - #Tier2 Travel Compliance Locals local is defined as 25 or 50 miles from facility, to be confirmed with the AM. Locals are not accepted Perm Staff Must be gone for 9 months before returning as traveler. RTO 7 days max. No RTO within the first 2 weeks. All RTO due at submission. Holiday work required, as needed. CNA License MN license in-hand required. RN / LPN License MN license in-hand or proof of application required (2-3 weeks via mail, 5-10 business days via email for processing). Saint Benedicts MN license in-hand is required. DO NOT submit if they have not applied for the license. Vaccines Medical / Religious exemption form required with submittal. Labs - Travelers are required to go to the lab within 48 hours of signing, or risk cancellation. Livescan Fingerprinting MN residents and non-MN residents must commit to completing this within 48 hours of signing, or risk cancellation. All travelers MUST complete fingerprinting in person in MN on the Monday prior to their Monday start . This is a MN State Requirement NO EXCEPTIONS.
