  • Location : Grand Blanc, MI 48439 – Michigan nursing jobs in a friendly city with scenic parks, excellent schools, and a strong sense of community.\n
  • Contract Length : 13 weeks\n
  • Schedule : 5x8 hour shifts per week (7 : 30am – 4 : 00pm), weekend rotation required\n
  • Guaranteed Hours : 40 hours per week\n
  • Estimated Weekly Salary : $2000-$2050 per week\n
  • Employment Type : Travel nursing contract\n
  • Setting : Acute inpatient case management\n

    • \n\nRegistered Nurse & LMSW Job Requirements\n \n

  • Minimum 5 years of acute inpatient case management experience, including acute discharge planning\n
  • Active Registered Nurse (RN) license in Michigan OR Licensed Social Worker (LMSW) license in Michigan\n
  • BLS certification through AHA\n
  • Experience with CarePort system\n
  • Excellent verbal communication and written documentation skills\n
  • Strong customer service, interpersonal, problem-solving, and analytical skills\n
  • Ability to acclimate quickly and work directly with patients on the floor\n
  • Preferred : Experience charting in AllScripts, GEMS / Sunrise; background in discharge planning, home health care, rehabilitative medicine, community health, or managed care\n

    • \n\nCase Management RN Responsibilities\n \n

  • Conduct comprehensive assessments of patient and family / caregiver biomedical, psychological, social, and functional needs\n
  • Develop personalized, patient-centered care plans to optimize care experience\n
  • Engage patients and families through advocacy, communication, health education, and resource facilitation\n
  • Utilize professional judgment, critical thinking, and motivational interviewing to help patients overcome barriers\n
  • Provide counseling and interventions related to treatment decisions and end-of-life issues, including Advanced Care Planning\n
  • Coordinate seamless and safe transitions between care settings\n
  • Advocate for appropriate delivery of services within the patient’s health plan benefit structure\n
  • Collaborate with the care team to co-manage patients with complex medical and social needs\n
  • Participate in process improvement activities to reduce risk and improve outcomes\n
  • Facilitate interventions in cases involving abuse or neglect\n
  • Support department goals and perform other duties as assigned\n
