About The Position

Job Title : RN Case Manager

Hours per Shift : 8 hours (8 : 00 AM - 4 : 30 PM, Monday - Friday)

Experience :

Minimum of 1 year in Medical / Surgical and / or Community Nursing

Prior experience in acute care discharge planning is preferred

Familiarity with discharge planning and utilization review process

2 years of experience required for non-U.S. candidates

License : Active NY RN license

Certifications : AHA BLS

Must-Have :

Certification for PRI, Screen, and MDS is preferred

Description :

This position is for a Registered Nurse specializing in case management.

The RN will work in patient management focused on effective care coordination.

The role requires collaborating with healthcare professionals for discharge planning.

Candidates must have a strong understanding of community agencies.

A minimum of one year of relevant nursing experience is required.

An active RN license in New York and Basic Life Support certification are mandatory.

This position has a standard schedule of Monday through Friday, no weekends or holidays.

Candidates residing within 50 miles of the facility are not eligible.
