Family Medicine Nuse Practioner

We are seeking a dedicated and compassionate Family Medicine Nurse Practioner to join our healthcare team. The ideal candidate will provide comprehensive care to patients of all ages, focusing on preventive care, diagnosis, and treatment of various health conditions. This role requires a strong commitment to patient service, excellent communication skills, and the ability to work collaboratively within a multidisciplinary team.

• Duties
• Conduct thorough physical examinations and patient assessments to diagnose and treat medical conditions.
• Develop and implement individualized care plans based on patient needs and preferences.
• Utilize EHR systems for accurate documentation of patient records and treatment plans.
• Provide preventive care, including immunizations and health coaching.
• Manage chronic diseases such as diabetes, hypertension, and obesity.
• Collaborate with specialists in areas such as cardiology, endocrinology, and gastroenterology for comprehensive patient care.
• Educate patients on health maintenance, disease prevention, and lifestyle modifications.
• Skills

The successful candidate should possess a diverse skill set including but not limited to :

• Proficiency in EHR systems for efficient patient management.
• Familiarity with coding systems (CPT & ICD) for accurate billing practices.
• Excellent communication skills for effective patient interaction and education.

Pay : $135,000.00 - $154,000.00 per year
