  • Wraparound Care Coordinator Program : Wraparound- Children and Family Services Reports To :   Program Supervisor Employment Type : Full Time(37.5 hours / week) Location : Onsite, - E 19th St,The Dalles, Oregon(Subject to work in Hood River, Sherman and Wasco Counties) Pay Range (Dependent on experience) : $27.28- 33.16 / hour FLSA Status : Not-Exempt      Description : To learn more about the Wraparound Program, please visit these links : https : / / youtu.be / X8_ia39661Q?si=NFTo9PRRhzuuu-B8 Home - Oregon Wraparound The Wraparound Care Coordinator works in collaboration with community partners and MCCFL Clinical Services Team to promote each client’s ability to effectively engage in services.
  • With the high prevalence of trauma among individuals receiving mental health services, the Wraparound Care Coordinator must possess demonstrated understanding of the effects of trauma on health, relationships, coping and other aspects of the lives of those we serve.
  • This role ensures that the Wraparound process is followed with fidelity and in accordance with the principals to empower youth and families to achieve their individualized goals.
  • They initiate parent and youth engagement, support team development to include natural and professional supports, facilitate regular meetings, develop, and implement individualized family plans of care plans that reflect the youth and family’s prioritized needs and preferences.
  • The Wraparound Care Coordinator will work with youth and families who are multi-system involved (juvenile justice, child welfare, substance abuse, special education, etc.) and must understand the complex systems of public services with the ability to forge strong connections to support client’s stabilization within their community.
  • Clients and family units may present with a variety of mental health challenges; trauma history, emotional and / or behavioral dysregulation, suicidality, disruptive attachment, and relationship difficulties.
  • Mid-Columbia Center for Living (MCCFL) is a Certified Community Behavioral Health Clinic (CCBHC) that specializes in providing services that are both trauma-informed and recovery-oriented to persons / consumers with mental illness and addictions.
  • Trauma-informed care includes safety, choice, collaboration, trustworthiness, cultural responsivity, and empowerment for both clients and staff.
  • MCCFL serves Hood River, Wasco, and Sherman Counties and utilizes a consumer-involved, consumer-engaged, and consumer-driven model.
  • MCCFL provides a whole-person care approach to adults and children to address both physical and mental health needs.
  • MCCFL offers a variety of programs including a mobile crisis unit, intensive case management, ongoing counseling, community support, supported employment, jail diversion, hospital discharges, hearings, and more.

What education and experience you need :

  • Any combination of education, licensure, and experience that demonstrates the ability to perform the position’s essential responsibilities.
  • This includes :

  • Must be certified as a Qualified Mental Health Associate (QMHA) by the Mental Health and Addiction Certification Board of Oregon (MHACBO) or register as QMHA-R within 14 days of hire.
  • Must successfully complete Oregon Wraparound Foundations Training within 6 months of hire.
  • Must receive and retain Oregon CANS 0.5 2.0 and Oregon CANS 6-20 2.0 certificates within 30 days of hire.
  • Two (02) years of relevant experience is preferred.
  • Bilingual fluency in Spanish / English is preferred.
  • Must pass all criminal history check requirements as required by ORS 181.536-181.537 and in accordance with OAR 407-007-0200 to 407-007-0370.
  • Must possess a valid Oregon or Washington Driver’s license, maintain an acceptable driving record, and be insurable for client service purposes and for travel between business offices, meetings and / or training.
  • What you'll do :

  • (This position may include any of the above-mentioned general responsibilities plus the following) : Engage high-need youth and their families in the Wraparound program.
  • Provide all services utilizing a strengths-based, family-focused, client-centered, trauma-informed, culturally humble approach that is integrated with other services.
  • Provide assistance in securing needed resources such as financial assistance, housing support, transportation, educational services, and treatment services as appropriate.
  • Work with youth and family members to assess strengths and needs across a number of life domains and participate in cultural discovery.
  • Complete Child and Adolescent Needs and Strengths (CANS) assessment regularly and uses results to inform care plan goals for each youth and family.
  • Collaborate with youth and family to identify significant others to be part of their Wraparound Team (the team ideally should be a combination of both professional and natural supports).
  • Engage youth and the Wraparound Team to develop an individualized, integrated, family and youth-driven, comprehensive care plan that meets the identified needs and incorporates identified strengths.
  • Help youth develop and access their own community supports.
  • Monitor progress on the youth’s care plan and reviews progress with the Wraparound Team on a regular basis.
  • Utilize agency clinical software systems to prepare current client records of treatment and reports as necessary and required.  Responsible for the maintenance and timeliness of all records, progress notes, and other required documentation.
  • Develop, in collaboration with the youth and family members, a crisis management plan that includes action steps and identifies specific responsibilities and communication procedures.
  • Assist family in implementing crisis plans as needed.
  • Coordinate Wraparound Teams and conduct team meetings with each youth and their team at least monthly, until no longer needed by the youth and family.
  • Provide linkage between the multiple systems within which youth are involved (e.g., child welfare, juvenile court, mental health, chemical dependency, special education, and other support systems).
  • Participate in decision making processes through collaboration with clinical services supervisors, managers, psychiatrists, other care coordinator(s), and staff members for emergency and stabilization of psychiatric patients, including emergency situations.
  • Attend Community Resource Team meetings as needed.
  • The work assigned is diverse and involves addressing new and unusual circumstances in which outcomes may negatively affect costs, employee morale or clients.
  • The work regularly involves a degree of unpredictability and disruption of planned tasks requiring a flexible time management approach.
  • Manage the mental demands of the position while monitoring and supporting client care and staff relationships.
  • Provide client transportation using agency vehicles as needed.
  • Regular and reliable attendance, maintain confidentiality, and engage in professional communication.
