Job descriptionDrive high-quality, safe, and cost-effective patient care in post-acute care communities Collaborate with other clinician interdisciplinary teams and follow CMS guidelines Work closely with hospice, palliative care, and home care agencies for safe patient transitions Ensure timely documentation and billing through our EMR (Gehrimed) meeting Medicare guidelines Develop and implement individualized care plans Ensure residents receive high quality medical care and oversight Board Certified internal medicine (IM) or family medicine (FM) Active Colorado license Active DEA Health insurance, 401K, short and long term disability, flex time (PTO), dental, and vision Highly competitive compensation Paid professional liability insurance with tail coverage 90-day onboarding program with MIPS training and support Educational opportunities with CME through TeamHealth and AMDA Well-developed infrastructure with extensive back-office support Growth opportunities into local and national leadership positions

Opportunity Overview :
- Location : Grand Junction, CO
- 15 -20 patients per day
- Flexible schedule Monday to Friday 2 days a week
- Day call on coverage days only
- Post-acute care inpatient setting

Compensation
Uncapped earning potential : our compensation is a productivity-based model (fee for service); expected compensation is estimated salary range of $108,000 to $130,000 annually with no cap on productivity income potential with full benefits.
