Psychiatric Nurse Practitioner (PMHNP) – Long-Term Care / Geriatric Settings
We are seeking an experienced and compassionate Psychiatric Nurse Practitioner (PMHNP) to provide high-quality mental health services to residents across several skilled nursing and rehabilitation centers in the Lower Hudson Valley and greater New York region.
This position is ideal for a clinician who enjoys autonomy, flexibility, and the opportunity to make a meaningful impact on the emotional and behavioral well-being of older adults.
Requirements
- Conduct comprehensive psychiatric assessments and ongoing evaluations for adult and geriatric residents.
- Formulate individualized treatment plans utilizing evidence-based approaches and pharmacologic strategies.
- Manage psychotropic medication regimens, monitor therapeutic effectiveness, and adjust as clinically indicated.
- Provide supportive counseling, behavioral interventions, and crisis stabilization when necessary.
- Collaborate with interdisciplinary teams—including primary care providers, social workers, and therapists—to optimize patient outcomes.
- Educate patients, caregivers, and staff about mental health conditions, coping mechanisms, and treatment adherence.
- Maintain thorough, compliant clinical documentation within the electronic health record system.
- Participate in care plan meetings, quality initiatives, and ongoing clinical supervision as required.
- Serve as a resource for staff education and consultation on psychiatric and behavioral management.
Qualifications
Current and unrestricted New York State Nurse Practitioner License with Psychiatric–Mental Health (PMHNP) certification.Prior experience in geriatric psychiatry, long-term care, or inpatient / outpatient behavioral health preferred but not mandatoryExcellent communication skills and the ability to collaborate effectively within multidisciplinary settings.Comfortable managing a mobile schedule across multiple facilities.Proficiency in EHR systems and basic computer literacy.Benefits
