365 Healthcare is seeking a LOCUM Physician - Medical Director for a full-time leadership and clinical position within a PACE (Program of All-Inclusive Care for the Elderly) model serving frail and elderly adults. This role combines medical oversight, interdisciplinary collaboration, and person-centered geriatric care.

The Medical Director provides clinical oversight, ensures high-quality participant care, and leads medical components of the PACE program. Responsibilities include participant assessments, care planning, provider supervision, and advancing quality outcomes across the program.
Key Responsibilities
Perform histories, physicals, and periodic medical assessmentsDevelop individualized treatment plansParticipate in applicant evaluations for program eligibilityCommunicate prognosis and care goals with participants and caregiversAttend interdisciplinary team meetings and care conferencesEducate participants and families on chronic disease and wellnessCollaborate closely with nursing, therapy, and social work teamsSupervise PCPs and APPsOrder and interpret labs, diagnostics, and document appropriatelyParticipate in provider on-call rotationSupport quality improvement and operational goalsRequired Clinical Expertise
