Special Equipment : VENTILATORS

REQUIRED : Servo I

PREFERRED : Puritan Bennett

Travel Compliance

RTO should be included at the time of submission (will be approved by unit manager on WOC; no other guaranteed time off. No time off will be granted during the first of week of the traveler s assignment), SSN, DOB, address, phone, and email.
